I have never seen a birth certificate, not even from places that claim to be queer-friendly, that had options other than "father" and "mother".

I have seen one birth certificate which said "parent 1" and "parent 2" but I'm not sure if it was a real birth certificate or a proposed template that died in votes before being adopted in the state.
